6.1. Foreword

6.1.1. Introduction

Welcome to the SMRU ANC (Antenatal Care) application. This application is designed to collect pregnancy data in an easy and safe way, but also to be a user friendly tool in the clinical setting.

This manual guides any user through all forms and fields. It serves as a comprehensive tool for using the application.

We invite everyone, to give feedback on the ANC Application and this manual.

6.2. Requirements

Requirements for using the ANC Application

Upon registration for every pregnant patient an ID card and a pink lemma is made. The card shows the woman’s name, her husband’s name, the registration date and a unique ID code, e.g. WPA-3001. This unique code is generated by the computer application and will never change during this pregnancy. The pink lemma stays with the woman, and will have pregnancy information relevant for other health providers in case she is referred. In addition, the pink lemma can serve as a medical note book for clinical information that cannot be captured in the computer application.

Each ANC department will have a laptop. To make the application user friendly colours are used for each department, both in the computer application as well as in the clinic itself. An example of a typical clinical setting:

6.3. Getting started

After switching on the laptop computer, the ANC application will be automatically opened. On a desktop computer, open the Firefox browser and type smru-anc/anc-app for the address. This page appears.

Depending on the site, there may be more than one database available. If so it is possible to choose the clinic that matches the database.

6.4. The Main Menu

This chapter explains the main menu bar at the top, showing

File This is to either create a new ANC patient file or to exit the application session and go back to the main login page. It is also possible to delete a form using this menu when the user has elevated privileges and is allowed to delete forms.

Appointments This is to open the appointment agenda of the different departments.

Browse Files This is to return to the main page.

Postpartum This menu is only shown for users with elevated privileges.

Settings This menu is only available for the Administrator user.

Help This is to show various information about the application.

6.4.1. File

The New sub menu is to create a new ANC patient file. By selecting this sub menu a dialog box will appear: Are you sure you want to create a new file? Select OK when you want to register a new pregnant woman in the application. The Quick Enrolment page will be shown. Select Cancel when you do not want to register a new pregnant woman into the application.

The Delete sub menu is only shown for users with elevated privileges. At the moment only the Administrator and IT users are allowed to delete a form. By selecting this sub menu a dialog box will appear: Are you sure you want to delete this entry? Select OK when you want to delete the form shown. Select Cancel when you do not want to delete the form.

The *Logout * sub menu is to exit the application. This can be used if you want to login as another user. You will return to the login screen.

6.4.2. Appointments

This shows the appointment agenda for the different departments. Depending on the user account used to login this page may show one or more departments. The Administrator user is shown all departments, but the Lab user is only shown the Lab department, while all other users are shown all departments except for the Lab department. Click on the appropriate link in the bar showing the departments just below the main menu to view the appointment agenda for that particular department.

6.4.3. Browse Files

This shows the main page of the appplication. All the pregnant women that are registered for today’s consultation are shown.

6.4.4. Postpartum

This menu is only shown for users with elevated privileges. The Submit sub menu can be used to submit a patient file or remind the ANC staff that something is missing. The Validate sub menu can be used to validate a patient file or to reject the validation request. The Archive sub menu can be used to archive a patient file or to reject the archivation request. At the moment only the Administrator, Doctor and IT users are allowed to submit or validate a patient file. And only the Administrator and Doctor users are allowed to archive a patient file.

6.4.5. Settings

This menu is only shown for the Administrator user. The Departments sub menu allows for making changes to the departments, like colour or name. At the moment there are 11 departments defined. See the introduction in the foreword for a description of the departments. The Users sub menu allows for making changes to the users, like password or privileges. At the moment there are 7 users defined. See the introduction in the foreword for a description of the users. The Check Database sub menu allows you to check the database for data entry errors. It will list all ANC codes that require attention due to some data inconsistency.

6.4.6. Help

The About sub menu shows the version information. The Legend sub menu shows a table explaining several features (e.g. colours). The Online Help sub menu is to view the online help in a separate tab.

6.5. Appointments

This page shows the appointments for each department. Click on a department in the centered line just below the main menu to view the appointments for that department. The departments for which appointments can be made are:

6.6. Browse Files

The main page in the application is the Browse Files page. Here all the women are shown that are registered for today’s consultation. When clicking on the row of a woman’s name in the main field her Antenatal Care Follow-up page appears. This page has two parts.

The Patient List at the left side of the page, showing the following columns

ANC Code This is the unique personal antenatal clinic code for this pregnancy. The ANC codes of the women who attended the ANC today appear in green color. The women who did not come today are standard blue.

Day This is the weekday of routine ANC. When a woman comes "in between" a different day is shown and the user knows that this woman had a special reason to come today.

PID This is the unique personal id code.

Woman This is the woman’s name.

Husband This is the husband’s name .

Outcome This is the date of outcome. If the date of outcome is completed in the application that date will appear in the column Outcome. When the outcome is more than 4 weeks ago this appears in red color.

Last Exam This is the date of the last weekly exam. If the last weekly exam was more than 4 weeks ago this date appears in red color.

EGA This is the estimated gestational age as of today. If the EGA is more than 34 weeks the number appears in orange color, as it is a reminder for the midwives that this woman is close to delivery and a delivery plan needs to be made. If the EGA is more than 42+0 weeks the number appears in red color. This is a warning for post dates and urgent action is needed.

EGA Type This is the method how the EGA was measured. The EGA Type can be CRL (Crown Rump Length), HC (Head Circumference), Say (what did the woman say), LMP (Last Menstrual Period), or SAC (gestational sac diameter).

Reason This is the reason why the woman has an appointment today. It shows the department for which an appointment was made for today.

M This flags if there are one or more malaria episodes.

MB This flags if there are one or more morbidities.

When clicking on a row in the Patient List the ANC Follow-up page of that woman is shown. See Chapter 7 for details on the ANC Follow-up page.

PID With this field a specific PID code can be searched for.

Name With this field a specific woman’s name can be searched for.

Patient day With this dropdown list the ANC day can be changed.

Post-it With this field the entered text can be searched for in all post-it’s.

Any Outcome over 28 days old is shown in red. Any Last Exam over 28 days old is shown in red. EGA’s over 34 weeks are shown in orange. However, EGA’s over 42 weeks are shown in red. The ANC code is shown in green when a weekly exam has been entered for today’s consultation.

6.7. Postpartum

The Postpartum menu gives acces to the Submit, Validate and Close pages. These pages are only shown for users with elevated privileges. At the moment only the Administrator, Doctor and IT users are allowed to submit or validate a patient file. And only the Administrator and Doctor users are allowed to close a patient file.

If you are not sure about submitting, validating or closing a patient file, you can click on the ANC code and the application will show the ANC Follow-up page. From that page you can access any form for that patient. Use the back link to return to the Postpartum page.

6.7.1. Submit

The Submit page is used to submit a patient file or remind the ANC staff that something is missing. It shows all patient files that are open and that have an outcome. When Remind is chosen, the user needs to enter a reason and a reminder appointment is automatically made for the Registration desk. The link turns red to indicate that a reminder has been sent to the ANC staff about possible missing data. When Close is chosen, the link turns green and the patient file will no longer be shown on the Browse Files page. Instead the patient file will be shown on the Validate page.

6.7.2. Validate

The Validate page is used to validate a patient file or to reject the validation request. It shows all patient files that have been submitted. When Reject is chosen, the user needs to enter a reason and a rejection appointment is automatically made for the Registration desk. The link turns red to indicate that the validate request has been rejected. Also, the patient file will now be shown again on the Browse Files page. When Validate is chosen, the link turns green and the patient file will now be shown on the Close page.

6.7.3. Close

The Close page is used to archive a patient file or to reject the close request. It shows all patient files that have been validated. When Reject is chosen, the user needs to enter a reason and an appointment is automatically made for the IT desk. The link turns red to indicate that the close request has been rejected. When Close is chosen, the link turns green and the patient file will no longer be shown on any of the Postpartum pages.

Patient files that have been submitted, validated or closed are no longer shown on the Browse Files page. However, it is still possible to access the patient file by typing the ANC code in the ANC code field of the Control section in the sidebar. Also, patient files that have been submitted or validated are still visible on the Validate and Close pages.

6.8. Settings

6.9. ANC Follow-up

This page shows the main menu with some extra menus and status information, the excerpt showing some basic patient details, the post-it for important notes about the patient, the appointments, the weekly follow-up and the sidebar.

You can open or close the post-it, appointments and weekly follow-up panels by clicking on the header of each panel. Closed panels have a header showing a plus sign and opened panels have a header showing a minus sign.

6.9.1. Main Menu

The main menu now includes the woman’s ANC code and the day of her routine ANC visits. The most right menu item is Options and shows some miscellaneous information.

6.9.2. Excerpt

The excerpt is the centered text just below the main menu, showing the woman’s name, her age, her husband’s name and his age. It also shows the Gravida, Para and the EGA in weeks + days. The methods of EGA estimation and the date this was estimated is shown inside parentheses.

6.9.3. Post-it

The Post-it shows important information that is not routinely entered into the application. This can be edited by clicking on the Edit link. Any text can be entered here, but it is recommended to keep this information short and very precise. Click Save to keep the text. This will appear in red in the application and will remain there until deleted by the user. Click Cancel to return to the antenatal care follow-up page.

Post-it text can be deleted by clicking on the Edit link. The text that is not needed anymore can be manually deleted and the post-it saved.

6.9.4. Appointments

The appointments panel can be shown by clicking on Appointments.

Quick appointments can only be made for the Belly check, Doctor, IT, Lab, PMTCT, Registration and Ultrasound by clicking on the specific button. An appointment line appears in this woman’s appointment list and automatically an appointment is created for today that is shown also in the Appointments page under the relevant department. With this option no extra information about the appointment is provided. For the Delivery room, IPD and Medic departments it is not possible to make a quick appointment, more information is required and the Appointment form is opened. When more information is needed for an appointment, for example a patient that needs an appointment with the medic because of fever, this information can be added to the appointment by clicking on the New link.

When accidentally a wrong appointment was created, that appointment can be deleted by clicking on the x in the last column.

Appointment form

The appointment form shows four fields:

Due on This is the time point for which the appointment is made. The default due time is today, but the appointment can be made for any estimated gestational age in weeks, for example EGA = 20 means that the appointment is made for this woman at EGA of 20 weeks.

Action This is free text to describe why the appointment is made. Here could be written: Fever and headache for 3 days. The Action label is orange which means that this field is required to be filled.

Department This is the department for which the appointment is made. The Department label is orange which means that this field is required to be filled.

Status This means whether the appointment has been done or not done and is by default Not done when the appointment is made.

After completing the four fields on the appointment page, press Submit to save the information and proceed. Select the back link to return to the previous page, the information will not be saved.

6.9.5. Weekly Follow-up

The weekly follow-up shows the next columns:

EGA This is the estimated gestational age in weeks + days.

Date This is the date of the consultation.

WT This is the woman’s weight in kilograms.

FH This is the symphysis fundal height in cm.

Fetus This is the position of the fetus, and can be H (Vertex), Br (Breech), Tr (Transverse) or NF (Not Felt).

FHB This is fetal heart beat in beats per minute.

SBP This is the systolic blood pressure in mmHg.

DBP This is the diastolic blood pressure in mmHg.

Temp This is the woman’s temperature in °C.

PR This is the woman’s pulse rate in beats per minute.

RR This is the woman’s respiratory rate in breaths per minute.

Liver This is the woman’s liver in cm palpable below the ribs.

Spleen This is the woman’s spleen in cm palpable below the ribs.

HCT This is the woman’s haematocrit in % (percentage).

MS/RDT This is the result of the malaria smear or the rapid diagnostic test and can be: Ab (Absent), Bad (Bad smear), Gam (Gametocytes only), Lab (Waiting for lab), ND (Came but not done), Neg (Negative) or Pos (Positive).

AB This is Why absent and can be: 1 (Go outside), 2 (Stay home), 3 (Sick relative), 4 (Stay SMRU IPD) or 5 (Other).

FS This is the number of Ferrous Sulphate tablets per week.

SUP This is whether the ferrous sulphate tablets were taken under supervision and can be: 1 (100% supervised), 2 (Partial 1 dose/day) or 3 (Unsupervised).

FA This is the number of Folic Acid tablets per week.

C This is the number of vitamin C tablets per week.

B12 This is the number of vitamin B12 tablets per week.

B1 This is the number of vitamin B1 tablets per week.

T This is the Tetanus vaccine given.

M This flags if there are one or more malaria episodes.

MB This flags if there are one or more morbidities.

S This flags if there are one or more ultrasounds.

6.9.6. Sidebar

Control

The Control panel allows navigation to another patient file by specifying the ANC code or the PID and clicking on Submit.

When entering a name or part of a name in the Name field and clicking on Submit all patient files are listed where the woman’s name matches the entered name.

Selecting a Patient day and clicking on Submit will list all patient files for that particular day.

When entering some text in the Post-it field and clicking on Submit all patient files are listed where the entered text is found in the post-it.

Studies

New

By clicking on one of these links the corresponding form is opened. The links are shown in orange when the required form has not been entered yet. The New Study Code link is shown in blue, because it is not required.

When the PMTCT test is waiting for a result this label is shown in orange. When the PMTCT test is positive the label turns blue again, and the program automatically sets the Chronic Maternal Illness risk factor and an appointment is automatically made with the Medic and the Doctor.

New ANC Forms

New Weekly Exam This form has to be completed at each antenatal visit.

New Study Code One or more study codes could be entered in this form. By completing this form a new Studies panel is shown in the sidebar at the right side of the screen.

New PMTCT By completing the PMTCT form a new Other panel header is shown.

Ultrasound When an Ultrasound form is completed, a blue flag appears in the ANC Follow-up page and a new Ultrasound panel is shown in the sidebar. This panel shows the ultrasound scans in chronological order as: Gestational age, e.g. 16+2, type of scan and (date of scan). Four forms can be completed: Ultrasound, Gestational Sac, CRL or Late Scans. By clicking on the link the form is opened.

Malaria When a Malaria form is completed, a red flag appears in the ANC Follow-up page and a new Malaria panel is shown. This panel shows the malaria episodes in chronological order as: Gestational age, e.g. 16+2, diagnosis and treatment. By clicking on the link the form is opened.

Morbidity When a Morbidity form is completed, an orange flag appears in the ANC Follow-up page and a new Morbidity panel is shown. This panel shows the morbidity episodes in chronological order as: Gestational age, e.g. 16+2, diagnosis. By clicking on the link the form is opened.

Other When a PMTCT Test, Urine Test, or Adverse Event form is completed, a green flag appears in the ANC Follow-up page and a new *Other*panel is shown. This panel is closed by default, but can be opened by clicking on it. The completed forms appear and can be opened by clicking on it.

Anaemia When an Anaemia form is completed, a purple flag appears in the ANC Follow-up screen and a new Anaemia panel is shown. This panel is closed by default, but can be opened and the completed forms: Episode, Symptoms, Risk Factor, Ferrous Compliance, G6PD, CBC, Thalassaemia, and Ferritin; all with the estimated gestational age. By clicking on the link the corresponding form is opened.

Outcome When a Delivery form is completed, a pink flag appears in the ANC Follow-up page and a new Outcome panel is shown. This panel is closed by default, but can be opened and the completed forms appear: it could be Outcome, Outcome Type, Mother, Partogram and Baby form, all with the estimated gestational age. By clicking on the link the corresponding form is opened.

Stillbirth When a Stillbirth form is completed, an orange red flag appears in the ANC Follow-up page and a new Stillbirth panel is created. This panel is closed by default, but can be opened and the completed forms appear: it could be Stillbirth, Stillbirth Test, Stillbirth Final Cause; all with the estimated gestational age. By clicking on the link the corresponding form is opened.

6.11. Basic Forms

After creating a new patient file the screen looks like:

The Post-it, Appointments and Weekly Follow-up panels are still empty because no form has yet been entered. All the orange links in the sidebar represent a form that needs to be completed. Each link will turn blue once the form is filled in and submitted.

The forms on the New panel can have more than one instance, e.g. a weekly exam for every visit, while the forms on the Forms panel can have only a single instance, e.g. there is only one enrolment form.

6.11.1. Single Instance Forms

Enrolment

The Enrolment form should be completed for each pregnant woman and has 7 sections: Date, Pregnant Woman, Parents of Pregnant Woman, Husband, Address, Obstetric History and Other. All fields that are required to complete appear in orange colour.

The Date field is pre-filled by the present date, but can be changed.

The Pregnant Woman section includes:

  • The PID (Personal Identification) is the finger print number.

  • Her Name, which is pre-filled according to the Name entered on the New File page

  • Her Age in years

  • Her Marriage number (drop down list including 0,1,2,3,4 and 5), default filled as 1

  • Her Place of outcome (radio buttons: Refugee Camp, Village in Thailand and Village in Burma).

The Parents of Pregnant Woman section includes the ethnicity of her mother and of her father (both drop down lists Sgaw Karen, Poe Karen, Muslim, Burmese and Other. When Other is selected, a new field appears to complete the other ethnicity).

The Husband section includes:

  • His Name, which is pre-filled according to the Name entered on the New file page

  • His Age in years

  • His Marriage number (drop down list including 0,1,2,3,4 and 5), default filled as 1

The Address section includes * her Address (as radio buttons: Refugee camp or Village - when Village is selected, a new field appears to complete the name code of the Village), * the Site of the border (as radio buttons Burma or Thailand) * House number * Section in the Village or Camp * How long does she live in this place, to be completed in years and months.

The Obstetric History section includes * the number of Pregnancies with SMRU (when a number greater than 0 is selected a new field appears to include the Previous ANC code) * The EGA by says (estimated gestation age) field as estimated by the pregnant woman (as radio buttons: Sure, Not sure and Don’t know - when sure or not sure are selected the field: EGA says should be completed, which shows the gestational age in weeks). * The Date of LMP (Last Menstrual Period) (as radio buttons: Sure, Not sure and Don-t know - when sure or not sure are selected the field: LMP date should be completed. A new field appears with a calendar, where year, month and day of months can be selected. When the correct date is selected the calendar can be closed by clicking X in the red box on the top right of the calendar field or by double-clicking the day number or. * Alive, which indicates the number of children that this woman delivered which are still alive. * Died, which indicates the number of children that this women delivered alive, but died later in life. * Stillborn, which indicates the number of children born dead after 28 weeks of gestation. * Abortion, which indicated the number of miscarriages before 28 weeks of gestation, or ectopic pregnancies, or molar pregnancies * Multiple pregnancies, which indicates the number of multiple pregnancies (twins, triplets, etc.) * Gravida; the number of pregnancies this woman had including the present pregnancy. This is default set as 1. The application calculates this number automatically by adding the numbers of Alive, Died, Stillborn, Abortion and Multiple pregnancies. * Para: the number of deliveries this woman had. This is set default as 0. The application calculates this number automatically by adding the numbers of Alive, Died, Stillborn and Multiple pregnancies.

Note: When viewing an existing enrolment form and the values entered for the Gravida and Para fields do not match the calculation, the application will give a warning by showing the fields in yellow colour.

The remaining fields include * Number of tetanus vaccinations; (drop down list including 0, 1, 2, 3, 4, 5 and 6), this is the number of tetanus vaccinations this woman had in her life. * Smoking this pregnancy (as radio buttons No or Yes). When Yes is selected two other fields appear: Age started smoking - in years, and Stopped smoking when discovered pregnant (as radio buttons No or Yes). * Can you read (as radio buttons No or Yes) - this should be tested by showing her a sentence in Burmese, English, Karen, or Thai language and ask her to read. * Can you write (as radio buttons No or Yes) - this should be tested by asking her to write a sentence in Burmese, English, Karen, or Thai language. * Does this patient take routine medication? (as radio buttons No or Yes). When a woman takes regular medications Yes should be selected, and when submit pressed the Routine Medication form appears:

Upon completion of the enrolment (± routine medication) form the application returns to the ANC follow up screen and the Enrolment (± routine medication) form field shows in blue colour.

Routine Medication form. This form consists of 4 fields: Medication, Indication, Provider and Start date which are all free text. In the Medication field the name of the drug should be filled - e.g. hydrochlorthiazide, in the Indication field the name of the disease for which these medications are prescribed - e.g. HBP, in the Provider field the organization/hospital that prescribed the medications - e.g. AMI and the Start date should show the date when this medication was started.
