Browse to https://forms.office.com.

Log in with your email account.
2. Usage
2.1. Create Form

Click New Form.
-
Type the form name in the Untitled form field.

Click Add new to add Choice, Text, Rating, Date, or etc.

Select the choice you want to add.

Required: Toggle the Required to turn on or off.

Optional: Click Style and select your favourite theme or color.

Click Collect responses.
-
Anyone: Choose Anyone can respond.

Internal only: Choose Only people in The Borderland Health Foundation can respond.

Optional: Check Record name.
-
Optional: Check One response per person.

Specific people only: Choose Specific people in The Borderland Health Foundation can respond.

Optional: Check Record name.
-
Optional: Check One response per person.

Type the user email address or group and press Enter.

Click Present.
-
Click Copy link to copy the link and send it to the users.
2.2. Download Responses

Select My forms.
-
Select the form that you want to download the responses.
-
Click Responses tab.
-
Select the arrow down on the Excel file and click Download a copy.
